Eligible applicants can do online
registration for applying for Passport. Using Internet Explorer
Browser is advisable for doing Online Registration.

Steps to be followed:
Open https://embassy.passportindia.gov.in/
and select country by clicking Czech Republic flag from the main
dashboard. Click ‘Register’ and select Embassy/ Consulate as
‘Czech Republic – Prague’. Complete the registration.

Login again using ‘User Login’.
Select ‘Apply for Ordinary Passport’ and proceed with the
application.

After filling the requisite
application form, a PDF file will be generated. Take a print out of
the online application form. Please ensure that specimen signatures
are affixed in black/blue ink, at the prescribed spaces.

The application form duly filled in
and signed is to be submitted, along with the requisite documents
(original and copy), at the Embassy on any working day (Monday to
Friday) between 0930 and 1200. List of holidays may
also be seen before coming to the Embassy.

In case of new
born baby, whose
either of the parents is Indian and they want to choose Indian
citizenship for the child, may apply for a fresh passport along with
Online Indian Citizenship Application Form. First
of all parents of the new born baby need to fill birth registration
form online, within six months of the baby’s birth at the
following link: https://indiancitizenshiponline.nic.in/Home2.aspx?formcode
with
upload of requisite documents such as birth certificate, passport
copies of parents, marriage certificate of parents. Thereafter, both
parents need to come to the Embassy along with the baby and copies
of online filled form and the uploaded documents and two
photographs. Simultaneously, parents need to fill online passport
application for the baby as per the usual procedure. In case of
minor child, an additional affidavit needs to be signed by both
parents at the Consular window of the Embassy. The
